Ontario Health atHome is seeking a dedicated Nurse Practitioner focused on delivering high-quality palliative care in Belleville, Trenton, and Picton. You will play a crucial part in managing pain, supporting psychosocial needs, and ensuring access to comprehensive care through collaboration with various healthcare professionals. Your expertise will guide patient-centered approaches, enhancing quality of life during critical times.

Key Responsibilities:

• Provide expert clinical care to palliative patients

• Conduct home visits and comprehensive clinical assessments

• Develop shared care plans considering diverse patient needs

• Utilize standard palliative care assessment tools

• Advocate for ethical decision-making in patient care
Requirements:

• Registration with College of Nurses of Ontario

• Certification as a hospice palliative care nurse

• Minimum 2 years of experience in Palliative Care

• Valid CPR-Level C & AED certification

• Ontario driver’s license and access to a vehicle
