Join Belleville and Quinte West Community Health Centre as a Nurse specializing in Chronic Disease Care to support individuals with various health conditions. This full-time position emphasizes education and comprehensive disease management.

As a CDPM Nurse, you will provide essential support and health education to clients managing chronic conditions such as COPD and diabetes. You will work collaboratively within an interdisciplinary team, ensuring that care is both trauma-informed and responsive to community health needs. Your contributions will directly impact program development and health service delivery.

Key Responsibilities:

• Conduct thorough assessments and health counseling for patients

• Collaborate in creating effective medical directives alongside Nurse Practitioners

• Apply a trauma-informed care philosophy in client engagement

• Develop, monitor, and evaluate chronic condition support programs

• Assist in general nursing duties as necessary

Requirements:

• Must be registered with the CNO as an RN or RPN

• RN candidates require a minimum of three years in chronic care settings

• CDE certification is highly regarded

• Proven experience in community health or primary care environments

• Background in program planning and delivery is beneficial
