As a full-time Registered Nurse, you will engage in scheduled visits across Belleville, Picton, Madoc, and Trenton. You’ll use your assessment skills to create and monitor personalized care plans while building long-term therapeutic relationships with your patients. This role fosters independence and offers a work-life balance through flexible scheduling, including every other weekend off.

Key Responsibilities:

• Conduct comprehensive patient assessments using appropriate tools

• Develop and monitor individualized care plans

• Build rapport with patients through collaborative engagement

• Update and maintain professional patient records

• Participate in on-call coverage to ensure patient needs are met

Requirements:

• Registered Nurse with CNO registration in good standing

• Valid driver’s license with vehicle access

• Experience in delivering community care is an asset

• Flexibility and reliability for travel to patient homes

• Bilingualism in English and French preferred
