This role involves working closely with physicians and nurse practitioners to deliver quality primary health care. You will perform assessments, administer medications, and monitor patients' progress while ensuring effective communication and collaboration within the health care team. Previous nursing experience in a primary care setting is preferred.

Key Responsibilities:

• Provide nursing services according to patient assessments

• Administer medications and monitor therapeutic effects

• Conduct follow-up communication with patients

• Maintain comprehensive patient care records in EMR

• Support interprofessional collaboration in care planning

Requirements:

• Degree in Nursing with current registration

• Membership with RNAO and certification in ACLS, BCLS

• Strong verbal, written, and interpersonal skills

• Valid driver’s license for home visits

• Availability for evening shifts
