Position: Lead Nurse for Home Care at FreedomCare
Location: Quinte West

Enhance patient care at Freedom Care as the Lead Nurse for our Colorado team. Oversee Care Managers while ensuring compliance with clinical guidelines and exceptional service delivery.

In this vital role, you will provide clinical leadership and support to the Colorado Care Management Team. You are expected to mentor Care Managers, review care plans, and ensure adherence to Colorado Medicaid requirements. Collaborate with Operations, Compliance, and Quality teams to drive improvements across clinical workflows.

Key Responsibilities:

• Mentor and guide Care Managers daily

• Review and approve care plans for accuracy

• Ensure compliance with Medicaid regulations

• Conduct routine chart audits and reviews

• Collaborate on new clinical programs and initiatives

Requirements:

• Active RN license in good standing

• BSN required; advanced degree preferred

• 3-5 years nursing experience required

• 2+ years in clinical leadership or management

• Experience developing SOPs or similar policies

Support the delivery of quality care through leadership, compliance, and mentorship in our Colorado home care team.
