Your primary duty as a Clinical Guide Population Health RN includes serving as a member's healthcare advisor. Under the supervision of the Sr. Clinical Manager, you'll facilitate effective care planning and support while navigating the complexities of the healthcare system. Engage with high-risk members and utilize analytics to improve care decisions.

Key Responsibilities:

• Perform evidence-based clinical triage and outreach

• Assist members in overcoming care access challenges

• Schedule appointments, referrals, and follow-ups efficiently

• Maintain thorough documentation of care provided

• Foster a member-centric approach to healthcare guidance

Requirements:

• Must hold an active RN license in a compact state

• Minimum 5 years of patient care experience

• Population health experience is essential

• Familiarity with triage nursing essential

• Background in chronic care management is preferred
