Key Responsibilities:
• Identify patient needs through comprehensive assessments
• Implement and adapt care plans as needed
• Foster therapeutic relationships to enhance patient care
• Maintain accurate and up-to-date patient records
• Be available for on-call care and support
Requirements:
• Active CNO registration as a Registered Nurse
• Access to a vehicle and valid driver’s license
• Experience in community health is beneficial
• Reliability and willingness to travel required
• Bilingual in English and French is an asset
