Key Responsibilities:

• Use assessment tools to identify patient needs

• Implement and adjust care plans based on assessments

• Participate in after-hours on-call support

Requirements:

• Active CNO registration as a Registered Nurse

• Driver’s license and access to a vehicle required

• Experience in community healthcare preferred

• Flexibility in scheduling needed

• Bilingual capability (English/French) is advantageous
