Entry-Level Case Manager in Denver

Join the mission of the Colorado Coalition for the Homeless as an Entry-Level Case Manager in Denver. Engage with families and individuals facing homelessness, driving toward sustainable solutions.

As a Case Manager I, you will work alongside health teams and community partners to provide vital support. This role requires developing and implementing care plans while managing client progress amid ongoing outreach efforts. You'll utilize motivational interviewing techniques and wrap-around care management principles to enhance client outcomes.

Key Responsibilities:

• Manage a caseload focusing on sustainability and engagement

• Coordinate care and services for clients' needs

• Monitor client progress and adjust care plans

• Liaise with community providers for integrated support

• Assist clients with applying for essential benefits

Requirements:

• 1-2 years relevant experience preferred

• High School Diploma or GED required

• Knowledge of local resources and social services

• Must maintain a valid Colorado driver's license

• Preferred: bilingual or multilingual abilities
