Care Manager Registered Nurse - Field; Metro East​/Southern IL

The Care Manager Registered Nurse will utilize critical thinking and judgment to facilitate appropriate healthcare outcomes for members by providing assistance with appointment scheduling and accessing benefits. This role is ideal for experienced nurses looking to make a life-changing impact on dual eligible populations.

• Pay: $66,575 - $142,576/year
• Location: Belleville, IL
• Brand/Division: Aetna
• Schedule: Full-time
• Travel: Up to 75% travel required

• Collaborate and inform the case management process.
• Assist members with appointment scheduling.
• Identify and assist with accessing benefits and education for members.
• Utilize care management tools and resources.
• Work independently and effectively communicate with teams virtually.
• Demonstrate proficiency with standard corporate software applications.
• Navigate multiple systems and applications simultaneously.
• Resolve common technical issues independently.

Education:

• Associate's degree required, bachelor's preferred

Experience:

• Minimum 3-5 years clinical practical experience
• Preferred 2-3 years in Care Management, discharge planning, and/or home health care coordination

Licenses &

Certifications:

• Active and unencumbered Registered Nurse license in the state of Illinois
• Certified Case Manager preferred
