Optum Home & Community Care seeks a House Calls Advanced Practice Clinician (Nurse Practitioner) to perform in-home health assessments for Medicare Advantage members. Work is field-based with a flexible 20–40 hour/week schedule, no on-call, weekends, or holidays to optimize work-life balance.
Active NP certification, an unrestricted state license, and reliable transportation are required. Compensation includes a $10,000 sign-on bonus and comprehensive benefits.
