Physical Therapist Assistant - SNF

Hillcrest Rehab Services is seeking a full-time Physical Therapist Assistant to join the Innovate Rehab & Wellness team at Hillcrest Health and Rehab in Bellevue, Nebraska. A PTA assists in delivering a plan of PT treatment established by a physical therapist using evidence-based techniques to promote movement, reduce pain, restore function and prevent disability.

• Collaborates with supervising physical therapist to plan transitions for patients between service lines.
• Performs patient treatments efficiently in accordance with the plan of care, progressing patients toward established goals.
• Completes accurate charging for services and provides documentation to support the provision of therapy services.
• Manages each patient’s plan of care through timely interdisciplinary collaboration and physician involvement as needed.
• Collaborates with and delegates appropriate tasks to technicians and other support personnel per federal and state regulations.

• Graduate from an accredited physical therapy assistant program.
• At least two years of experience performing as a PTA in a long-term care, post-acute, hospital, home care or similar setting preferred.
• Must be licensed in the state of Nebraska or compact state as a physical therapist assistant.
• BLS certification by the American Heart Association required.
