- Conduct patient rounds as assigned.
- Assist patients with bathing, oral care, dressing/undressing, toileting, perineal care, grooming, eating and taking/recording vital signs in accordance with facility policies.
- Assist with daily housekeeping duties – making beds, taking out trash, removing trays and dirty dishes, etc.
- Assist with reminding and/or assisting residents to the dining room and daily activities.
- Assist with special events as needed.
- Maintain your assignment as directed.
- Demonstrate competency in the prevention of foodborne illness and safe food handling.
- Assist with activities of daily living as designated in each patient’s care plan.
- All calls for assistance must be responded to promptly. If an assigned caregiver cannot answer the call promptly, they are to request a replacement or immediately advise their supervisor.
- Assist in emergency situations. Call 911 and request supervisor/med tech assistance. Complete Incident Report and give completed documentation to Nursing Supervisor and/or Executive Director.
- Ensure correct recording of all resident ADL’s before leaving shift. Receive/give verbal updates at shift change.
- Perform all other duties as assigned by Med Tech, Supervisor, or Executive Director within your scope of practice.
- Security checks as assigned within your scope of practice. Monitor and ensure the general wellbeing of all residents. Be familiar with current individual resident service plans.
- Communicate changes in condition (medical, emotional, physical) immediately.
- Perform all tasks in a professional, timely and caring manner while maintaining a positive and cheerful attitude.
- Consistently work cooperatively with patients, nurses, other nursing assistants, supervisors, physicians, families, consultant personnel and ancillary service providers.
- Provide a safe and secure environment while ensuring a high degree of confidentiality and respect for resident’s privacy. Be knowledgeable and aware of HIPAA regulations.
- Follow established performance standards and perform duties according to nursing service policies and procedures.
- Participate in interdisciplinary team and other facility meetings as assigned.
- Proficient computer skills to manage the electronic health record (EHR).
- Ability to manage information electronically and transfer records appropriately.
- Ability to work in an electronic environment.
- Must possess the ability to read, write and speak English.
- Must possess strong oral, written and telephone skills.
- Able to lift 50 pounds unassisted.
- Must be in good health and physically/mentally capable of performing assigned tasks.
- Certified in First Aid/CPR.
- Obtain any permits or training required for the job.
