Senior Primary Care NP​/PA: Team-, Patient-Centered

Humana Center Well Senior Primary Care seeks a Nurse Practitioner or Physician Assistant to deliver comprehensive, value-based care for seniors at our State Ave. clinic. You will work with physicians, pharmacists and care teams to coordinate referrals, hospital coordination, DME and home health services.

You will document care with support from coders, participate in on-call rotations, and contribute to center growth plans while upholding high-quality standards.
