The Registered Nurse is responsible for providing nursing care to patients, supervising care treatment and services of patient care provided by LPNs, CNAs and other staff on assigned unit and shift. Develops, implements, and evaluates an individual plan of care for each client. Performs assessment and re‑assessment of patients and communicates appropriate information with the interdisciplinary team. Reports client concerns and changes in condition to supervisor and/or physician in a timely manner.
Compensation Range: $37.50/hr – $56.00/hr
Schedule: 7:00 PM – 7:30 AM
- Provides and coordinates care by assessing physical and behavioral health needs of clients, develops and implements nursing care plans, maintains medical records and educates clients and families about various conditions while ensuring client safety at all times.
- Assesses client status on an ongoing basis and documents pertinent information.
- Ensures nursing treatment plans are specific to assessed needs and the initial treatment plan is completed upon admission.
- Takes physician’s orders (verbal or via telephone) accurately, documents appropriately, and transcribes and reviews physician’s orders; ensures orders are legible.
- Accurately administers medications per ordered note and documents accurate count of controlled substances; adjusts medications under provider supervision as needed and monitors lab results.
- Notifies physician of significant changes in status such as difficulties with detox, medical emergencies or changes in medical status regarding response to medications.
- Supervises self‑administration of medications using safety initiatives of two means of client identification; documents and reports effects of scheduled and PRN medications.
- Maintains accountability of medication administration, recording, control and security, including narcotics.
- Completes admission and discharge processes efficiently and accurately as directed.
- Confers regularly with the treatment team (nurses, therapists, program managers, physician) regarding client care and attends unit activities such as therapy groups, family meetings, staffing meetings, etc.
- Provides education on medication usage, common side effects, patient rights, and other topics necessary for optimal client care.
- Monitors client acuity, census, staffing needs, and arranges coverage as needed according to guidelines.
- Ensures thorough, accurate documentation for all clients on each shift and completes client assignment sheets daily.
- Reports to oncoming staff, supervisors, and physician.
- Provides coverage in emergency services as needed.
- Graduation from an approved/accredited school of nursing.
- One or more years of nursing experience within the clinical area of service provided by the facility preferred.
- Experience in a behavioral health/psychiatric/addiction recovery environment preferred.
- Valid current RN license from the state or from a compact state, where applicable.
- Certified in Psychiatric Nursing preferred.
