Community Navigator (Master’s-Level Clinician)
Brigid Collins Family Support Center | Full-Time, 40 Hours per Week | Whatcom County
Our MissionBrigid Collins Family Support Center partners with families to support the safety, healing, and well-being of children through compassionate, relationship-based, and community-centered services.
What You’ll Do- Serve as the primary liaison for behavioral health referrals from primary care providers and clinic teams
- Receive, triage, and coordinate referrals; gather relevant clinical and psychosocial information from families and care teams
- Conduct evidence-based screenings and assessments to identify symptoms, strengths, risk factors, and level of need
- Provide clear clinical impressions and recommendations aligned with best practices and family preferences
- Offer brief consultation, coaching, and psychoeducation to caregivers around child mental health concerns and next steps
- Partner with families using a strengths-based, trauma-informed, and culturally responsive approach to reduce barriers to care
- Facilitate warm handoffs and referrals to outpatient therapy, psychiatry, school-based supports, community agencies, and crisis services
- Track referral progress and ensure closed-loop communication with referring providers
- Collaborate closely with medical providers, clinic staff, and community partners to coordinate care and improve access
- Maintain timely, accurate clinical documentation in accordance with organizational and regulatory requirements
- May carry a partial caseload
- Other relevant duties as assigned
- Works closely with primary care teams and external referral partners.
- May include a mix of in-clinic, phone/video, and community-based coordination.

- $30.00–$32.57/hour (Associate License)
- $34.00–$38.64/hour (Independent License)
- +$1/hour bilingual add-on (Spanish or Mixteco)

Required:
- Ability to consistently meet established productivity, documentation, and quality benchmarks.
- Experience with care coordination/navigation in a community health setting.
- Strong understanding of child development, trauma, and family systems.
- Ability to maintain a required caseload and consistently meet expectations.
- Skills in child and family assessment, treatment planning, and therapeutic intervention.
- Ability to communicate clearly and compassionately with children, caregivers, and partner agencies.
- Ability to maintain professionalism, confidentiality, and ethical practice in high-stress situations.
- Ability to work within a multidisciplinary team and navigate complex systems.
- Strong organizational, documentation, and time-management abilities.
- Humility and ability to engage diverse, multi-stressed families.
- Willingness to adjust schedule to meet with families at times that are convenient for their schedules.
- Ability to travel throughout Whatcom and/or Skagit Counties to serve families.
- Master’s degree in social work, counseling, MFT, psychology, or related field
- Washington State behavioral health license (associate or independent)
- Strong assessment, documentation, and care coordination skills
- Community-based or integrated care experience preferred
- Valid WA driver’s license and ability to travel locally
