Job Summary- Provides patient care in a Rehabilitation Hospital, LTC, Assisted Living
- Works on a per diem (as needed) basis
- Completes 8-hour shifts as scheduled
- Assists patients with rehabilitation and recovery activities
- Collaborates with interdisciplinary teams to deliver optimal care
- Adheres to hospital policies, procedures, and safety guidelines
- Maintains accurate and timely documentation of patient care
- Communicates effectively with patients, families, and staff
- Supports patient progress towards rehabilitation goals
- Ensures a safe and therapeutic environment for all patients
