- Support residents and families throughout their stay and discharge planning process
- Coordinate care and resources with residents, families, and community partners
- Participate in interdisciplinary care planning and resident assessments
- Help ensure residents receive the support and services they need to thrive
- Promote resident rights, dignity, and quality of life
- Bachelor's degree in Social Work
- Two years of social work experience required
- Skilled nursing or healthcare experience required
- Knowledge of state and federal regulations
- Strong communication, assessment, and documentation skills
- Ability to work collaboratively with residents, families, and interdisciplinary teams
