Peace Health in Bellingham, WA is seeking a RN Home Care‑ Home Health for a Part Time, 0.80 FTE, Day position. The salary range for this opening is $40.98 – $77.86
. The hiring rate will depend on factors such as education, training, work experience, and other applicable considerations.
- Perform initial evaluation of patient/home and apply admission criteria to determine if the patient is a safe candidate for home health/hospice services.
- Provide skilled nursing care and skilled education/instruction to patients and caregivers according to the home health/hospice plan of treatment.
- Conduct physical assessments, develop, coordinate, modify and manage written individualized plans of care as ordered by the primary physician.
- Develop and clinically manage the Home Health Aide’s care plan according to the patient’s evolving needs.
- Coordinate care, collaborate and communicate with other members of the interdisciplinary team to ensure appropriate and smooth flow of care/services.
- Perform discharge planning appropriately toward patient‑specific goals/outcomes and evaluate interventions and outcomes of patient care by reviewing long and short‑term goals established in the plan of care.
- Provide accurate, complete and timely clinical documentation in compliance with regulatory, quality and agency guidelines.
- Assist with the plan and implementation of family bereavement services for patient deaths.
- Perform other duties as assigned.
- Accredited School of Nursing required.
- Bachelor of Science in Nursing preferred.
- 2 years acute clinical or home health/hospice nursing experience required.
- 1 year home health experience preferred or will orient to point of competency.
- Demonstrated knowledge of and ability to apply age‑specific principles of growth and development and life stages to meet each patient’s needs.
- Registered Nurse in state of practice required.
- Driver’s license and proof of auto insurance required.
- Upon hire basic life support required.
- Basic computer skills including MS Office applications required.
- Well organized, flexible and able to function independently required.
- Cooperative working skills, leadership skills and interpersonal skills required.
Will accept valid driver’s license in US or Canada and proof of auto insurance.
Working Conditions Lifting- Patient handling no greater than 35 lbs. without assistive equipment or devices (NIOSH).
- Fine motor skills required to grasp and control medical equipment and perform precise procedures.
- Push/Pull:
Frequently up to 45 lbs. force (e.g., medical carts). - Hands‑on CPR at least 20 minutes using 100‑125 lbs. of force (National Association of EMS Physicians and AHA); frequency should be seldom or occasionally.
- Ability to move around area with frequent sitting.
- Bending, stooping, squatting, reaching, kneeling frequently.
- Lifting non‑patient up to 20 lbs. occasionally and 5 lbs. frequently.
- Exposure to biohazard, body fluids and airborne particles.
- Must be able to complete tasks in a noisy environment.
- Vision and hearing required within normal limits (glasses, contacts, hearing aids permitted).
- Ability to communicate and exchange accurate information.
