Position: Community Residence Counselor\

At The Pavilion, under the direction of the Nurse Manager, Clinical Coordinator, and Charge Nurse, the Community Residence Counselor (CRC) provides direct patient care and participates in the therapeutic milieu within the scope of defined practice.

Essential Functions
  • Provides direct patient care in accordance with the current treatment plan, under the supervision of the Nurse Manager, Clinical Coordinator, and Charge Nurse.
  • Provides and supervises community activities according to needs, which may include driving and transporting patients to and from activities in hospital vehicles.
  • Remains awake and alert at all times while on duty.
  • Assists patients in maintaining and keeping various appointments.
  • Maintains clear, concise, and timely documentation and shift notes.
  • Establishes helpful relationships with patients while maintaining professional boundaries.
  • Participates in patient rounds, status checks, or team meetings as per program guidelines.
  • Completes assigned patient care tasks according to program guidelines and/or daily assignment sheets.
  • Works effectively to manage the therapeutic milieu.
  • Provides patients with direction and supervision as needed.
  • Follows established hospital and program policies, procedures, and protocols.
  • Communicates information regarding the patient milieu and therapeutic environment to appropriate staff in a timely manner.
  • Reports observations related to medication side effects, physical and emotional changes to appropriate medical staff promptly.
  • Encourages patients in their activities of daily living.
  • Assesses sleep patterns and assists patients in developing more appropriate sleep cycles, notifying the clinical team as necessary.
  • Completes mandatory annual in-service requirements, including CPR and de-escalation training.
  • Attends and participates in rounds and case conferences when appropriate.
  • Possesses the physical ability to participate in patient activities.
  • Performs other duties as assigned.
  • Can this role accept experience in lieu of a degree?

    Experience in the relevant field may be considered in lieu of a degree, subject to the organization's policies.

    Licenses and Credentials

    Medication Administration Program Certified (MAP - Massachusetts) preferred.
