• Provide competent, courteous, and respectful evaluation and treatment of patients for difficulties with mobility, function, strength and positioning in accordance with the APT scope of practice and licensure laws.
• Communicate and collaborate with families, physicians, and other health team members; maintain professional documentation of services in the medical record.
• Conduct in-services and training for facility staff on restorative and functional maintenance programs.
• Report to the Therapy Program Manager and ultimately to the Director of Operations; model and promote the concept of professional services to internal and external customers.
• Maintain career development through continuing education and training opportunities.
• Demonstrate CRI values (Commitment, Respect, Integrity) in providing therapy.

• B.S., Masters, or DPT in Physical Therapy.
• Licensed in the state(s) where services are provided as a Physical Therapist.

• Evaluate patients for rehabilitation services within 24 to 48 hours of physician’s orders.
• Develop effective treatment plans and implement them through patient care.
• Treat patients according to the treatment plan of care.
• Supervise the Licensed Physical Therapy Assistant and interact with staff, patients, and families.
• Communicate with physicians, clinical managers, and other health team members regarding patient progress and plans.
• Participate in Plan of Care Meetings and Interdisciplinary Team Meetings.
• Meet or exceed the minimum productivity standard established by CRI.
• Participate in in-service training for facility staff.
• Record treatment notes, progress notes, discharge summaries, and complete daily billing per policy.
• Instruct patients’ families and/or facility staff in maintenance programs.
• Participate in discharge planning and recommend needed equipment to facilitate care.
• Report equipment problems to facility personnel.
• Act as a clinical supervisor for affiliating Physical Therapy and Physical Therapy Assistant programs for area universities and colleges.
• Represent Concept Rehab with professional image and good judgment.
• Perform special projects and other duties as assigned.
