Access North has a Full-Time Case Manager position available in the Duluth area. This position has the potential to become a hybrid position working both on-site and remotely.
Note:
This position will be a 32-hour work week, Monday through Thursday, with Fridays off. This means it will still be considered Full-Time and eligible for Full-Time benefits.

Responsibilities- Oversee operational and administrative functions necessary to achieve the goals and objectives of Access North's in-home supports for 245D & PCA consumers.
- Network, market, and conduct outreach with the county, other service providers, agencies, and community organizations in the service area.
- Supervise and train staff in relation to DSP compliance.
- Document and maintain accurate records showing the services provided and time spent by the DSP.
- Maintain relationships with county social workers and waiver specialists throughout the service delivery area.
- Collaborate with HR to identify and fill DSP needs across the service area.
- Ensure DSP training related to 245D & PCA compliance and self-directed/person-centered practices.
