Are you a licensed Pharmacist looking to make a meaningful impact while enjoying the pace and lifestyle of Southern Oregon? Serenity Med Search is partnering with a well-established healthcare organization to recruit a dedicated Ambulatory Care Pharmacist for a full-time, direct-hire position.

You’ll join a collaborative pharmacy team supporting both inpatient and outpatient services, with the opportunity to contribute to safe medication management, clinical initiatives, and patient education.

• Provide direct patient care through collaborative practice agreements or provider referrals for chronic disease management (e.g., diabetes, hypertension, anticoagulation, heart failure).

• Conduct comprehensive medication reviews, assess adherence, monitor outcomes, and adjust therapies as appropriate.

• Document interventions, care plans, and outcomes in the electronic health record in accordance with regulatory and hospital standards.

• Serve as a resource to ambulatory care providers and clinic staff regarding medication therapies, interactions, and evidence-based guidelines.

• Participate in care team huddles, case reviews, and quality improvement initiatives.

• Educate patients and caregivers on disease state management, medication use, and self-monitoring strategies.

• Monitor and address medication safety issues, adverse drug reactions, and medication-related problems.

• Precept pharmacy residents and students as assigned; contribute to training and development efforts.

• Support initiatives aimed at reducing hospital readmissions and improving clinical metrics for ambulatory populations.

• Doctor of Pharmacy (Pharm
  
  D) from an accredited college of pharmacy

• Completion of an ASHP-accredited PGY1 residency required; PGY2 in Ambulatory Care or relevant experience preferred

• Current pharmacist license in good standing (or eligible) in Oregon

• Board certification (e.g., BCACP) strongly preferred or willingness to obtain within 1 year

• Experience working in team-based care settings with strong patient communication skills

• Proficiency in electronic health records and clinical documentation
