Are you a licensed Pharmacist looking to make a meaningful impact while enjoying the pace and lifestyle of Southern Oregon? Serenity Med Search is partnering with a well-established healthcare organization to recruit a dedicated Ambulatory Care Pharmacist for a full-time, direct-hire position.
You’ll join a collaborative pharmacy team supporting both inpatient and outpatient services, with the opportunity to contribute to safe medication management, clinical initiatives, and patient education.
Key ResponsibilitiesProvide direct patient care through collaborative practice agreements or provider referrals for chronic disease management (e.g., diabetes, hypertension, anticoagulation, heart failure).
Conduct comprehensive medication reviews, assess adherence, monitor outcomes, and adjust therapies as appropriate.
Document interventions, care plans, and outcomes in the electronic health record in accordance with regulatory and hospital standards.
Serve as a resource to ambulatory care providers and clinic staff regarding medication therapies, interactions, and evidence-based guidelines.
Participate in care team huddles, case reviews, and quality improvement initiatives.
Educate patients and caregivers on disease state management, medication use, and self-monitoring strategies.
Monitor and address medication safety issues, adverse drug reactions, and medication-related problems.
Precept pharmacy residents and students as assigned; contribute to training and development efforts.
Support initiatives aimed at reducing hospital readmissions and improving clinical metrics for ambulatory populations.
Doctor of Pharmacy (Pharm
D) from an accredited college of pharmacyCompletion of an ASHP-accredited PGY1 residency required; PGY2 in Ambulatory Care or relevant experience preferred
Current pharmacist license in good standing (or eligible) in Oregon
Board certification (e.g., BCACP) strongly preferred or willingness to obtain within 1 year
Experience working in team-based care settings with strong patient communication skills
Proficiency in electronic health records and clinical documentation
