Department Summary

The St. Charles Health System Pharmacy provides a broad range of inpatient and outpatient pharmaceutical care services to ensure safe and effective medication use.

Position Overview

The Clinical Pharmacist provides pharmaceutical care as a complete package across the hospital medication process in order to increase the effectiveness and quality of patient care. This position does not directly manage any other caregivers.

Essential Functions and Duties

Provides specialized pharmaceutical services to improve drug usage and therapeutic outcomes including, but not limited to, advising physicians on issues concerning drug therapy, the inherent toxicity of drugs and their side effects, as well as assisting in the prescription of appropriate doses.

Coordinates the activities of assigned pharmacy technicians engaged in the preparation, labeling and delivery of medications and other pharmaceutical supplies in accordance with physicians' prescriptions. Ensures compliance with federal, state and local laws and regulations pertaining to the dispensing of drugs and controlled drugs and the maintenance of required records.

Mentors and trains others in training including practical experiences.

Maintains liaison relationships with medical and nursing staff. Provides timely information pertaining to pharmaceutical supplies, drug usage and compatibility, state and federal regulations regarding drug controls, and Joint Commission Standards.

Conducts and evaluates medication histories, assesses compliance and suggests modifications so as to achieve desired outcomes; instructs patients in the proper use of prescribed drugs and makes Patient Care Rounds with physicians to evaluate patient progress.

Individualizes medication regimens using sound principles, accounting for pharmacodynamic and pharmacokinetic variations in drug absorption, distribution, metabolism and elimination with responsibility for establishing and continually improving the delivery of Pharmaceutical Care to patients within areas of direct responsibility and assisting others in the department with the same.

Extracts pertinent information from patient/other sources for the patient's medical management plan.

May perform additional duties of similar complexity within the organization, as required or assigned.

Registration by the Oregon Board of Pharmacy. AHA Basic Life Support for Healthcare Provider certification (not required if working exclusively in Specialty Pharmacy).
