Pay range (depending on experience):
Schedule/Shift: Flexible
Compensation: Starting at $38/hour, open to discussion
Responsibilities- Conduct psychosocial assessments of patients to understand their needs within the context of their illness and environment.
- Develop and implement tailored intervention plans based on assessment findings.
- Maintain accurate clinical records for all patients referred to social work services.
- Offer information and referral services to patients, families, and caregivers regarding practical and environmental needs.
- Serve as a liaison between patients, families, caregivers, and community agencies to facilitate access to support services.
- Graduation from a Master’s program in social work accredited by the Council on Social Work Education.
- Active license as clinical social worker by the Board of Behavioral Sciences.
- If you reside in Oregon or Washington an LCSW or AWS is required.
- Minimum of one (1) year of experience in healthcare, with preference given to candidates with home health care experience.
- Excellent verbal and written communication skills, along with strong organizational abilities.
- Current BLS, hands‑on CPR certification.
- Valid driver's license and reliable, insured transportation.
