Monday - Thursday, 8 am - 5 pm;
Friday, 8 am - 12 pm - 36 hours/week

• 15 patients per day
• Geriatric patient population, ages 65+
• Long term care experience required
• Geriatric experience required
• SNF experience preferred
• Palliative care skills preferred
