Registered Nurse; RN-Case Manager, Health Resource Management

The Health Resource Manager is responsible for providing case management services for SVMC sub acute patients. The case manager is responsible for coordinating a patient-centered plan of care to meet individual needs by:

• Completing intake interviews to identify a holistic plan of care; facilitating multiple aspects of care coordination with the patient, family/caregiver, providers and other community resources prior to discharge
• Developing a patient transition plan, providing patient/caregiver education and proactively advocating for patient needs and goals across the interdisciplinary team to ensure an approach that is sensitive to each specific patient and family’s needs
• Assisting providers in determining patient’s level of care in accordance with Inter Qual criteria
• Actively seeking out collaboration with providers to improve overall quality and efficiency of care

Graduation from an accredited school of nursing and a Bachelors of Science of Nursing required. (If matriculated into BSN program, person is eligible)

Three-Five years of clinical nursing experience required.

Prior experience in utilization review, discharge planning and/or case management preferred.

VT Nursing License required by date of employment.

Case Management Certification required within 2 years of employment.
