BASIC PURPOSE OF THE JOB:

  • The RN Bridge to Primary Care clinical coordinator partners with primary care providers, the patient/family, and their care team to assist and navigate high risk, complex, and chronic care patients across the care continuum.
  • Navigates the continuum of care by serving as a clinical resource, assisting with access issues, coordinating transitions with providers and the patient.
  • Responsible for oversight of clinical staff and nursing care delivery, quality of care, regulatory compliance, clinical staff professional development and peer review.
  • Collaborates with medical providers, patients & families, community partners and stakeholders.
  • Performs timely and accurate information collection, appropriate care advice/clinical care delivery including accurate dispositioning, and comprehensive documentation.
JOB QUALIFICATIONS:
  • Minimum Education:
    • Graduation from an accredited school of nursing required.
    • Bachelor’s of Science in Nursing strongly preferred.
  • Minimum

    Work Experience:

    • Prior clinical nursing experience required.
    • Care coordination and transitions of care experience strongly preferred.
    • Primary care or emergency department triage background or call center experience helpful.
    • Candidates with experience in customer service within the health/medical industry.
    • Minimum of 3‑5 years successful RN practice with adult and pediatric populations required.
  • Required Licenses/

    Certifications:

    • Unencumbered RN license in VT, NY, and MA required.
      • Ability to obtain a valid Vermont State Registered Nursing License by date of employment required, NY & MA to follow.
      • Health Care Provider BLS certification required within 90 days of hire.
REQUIRED SKILLS, KNOWLEDGE, AND ABILITIES:
  • Ability to maintain valid Registered Nurse license required.
  • Knowledge of Vermont, New York, & Massachusetts Nurse Practice Act required.
  • Ability to maintain current Health Care Provider BLS certification required.
  • Ability to successfully complete annual mandatory education requirements in accordance with educational policies.
  • Ability to utilize the nursing process in providing patient care required.
  • Ability to effectively document patient information and the nursing process required.
  • Knowledge of and ability to apply the principles of evidence‑based nursing care required.
  • Ability to demonstrate necessary competencies for specific jobs required.
  • Ability to participate in departmental QI activities required.
  • Ability to consistently display professional, ethical standards of care consistent with SVMC Nursing Magnet designation required.
  • Must be familiar with staffing and scheduling, patient flow and placement, and staffing ratios, acuity systems and nursing skill mix.
  • Knowledge of team members' roles, scope of practice and responsibilities required.
  • Ability to work in a fast paced, multi‑tasking medical office required.
  • Strong written, verbal communication, excellent teaching, customer service skills.
  • Detail oriented, strong documentation skills, clinically astute.
  • Possess critical thinking skills and be able to problem solve independently.
  • Familiarity with community resources and hospital processes, structure and function.
  • Ability to carry clinical and technical skills across various clinical situations.
  • Understands and is able to coordinate a variety of care‑providers and schedules to maximize the patient and family experience.
  • Effective relationship management, shared decision making, and structural empowerment.
  • Complex program/project planning, implementation and evaluation.
  • Knowledge of departmental/organizational emergency preparedness plan & procedures required.
  • Ability to effectively use the SVHC intranet to access & implement the policies, procedures, protocols, and job aides required.
  • Ability to effectively operate a variety of Microsoft Office, Outlook, Meditech HIS, Athena EHR, and phone system software required.
