Registered Nurse; RN-Annual Wellness Visit Nurse

The Annual Wellness Nurse assists and supports in the coordination, development, implementation, and evaluation of the standards and practice of Primary Care Annual Wellness Exams. This position performs primary care annual wellness exams, following guidelines and meeting documentation requirements. This position will also perform outreach as necessary, support development of annual wellness exam protocols and standing orders.

This position will require some travel to off site offices.

Job qualifications include: graduation from an accredited school of nursing which is required. Matriculation into a Bachelor’s Degree in Nursing preferred. Five years of clinical experience required with at least one year of clinical experience in a medical office setting preferred. Valid NY and VT Nursing license required. BLS required.
