We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our healthcare team. In this role, you will provide direct patient care, assist with activities of daily living, administer medications, and support patients' physical and emotional well-being. You will work closely with registered nurses, doctors, and other healthcare professionals to deliver high-quality care and ensure positive patient outcomes.
Key Responsibilities- Provide direct care to patients, including taking vital signs (blood pressure, temperature, pulse, and respiration), assisting with personal hygiene, and supporting activities of daily living such as dressing, bathing, and eating.
- Administer prescribed medications, monitor patients for side effects or adverse reactions, and report any concerns to the healthcare team.
- Assist in the care and dressing of wounds, ensuring proper healing, infection prevention, and monitoring for complications.
- Observe and document patient conditions, including physical and emotional well-being, in medical records. Report significant changes to the healthcare team promptly.
- Collaborate with registered nurses, physicians, and other healthcare providers to develop and implement comprehensive patient care plans.
- Provide support and education to patients and their families regarding health conditions, treatments, and self-care instructions.
- Maintain a clean and safe environment for patients, adhering to infection control and safety protocols.
- Assist with the coordination of patient care activities and follow up on prescribed treatments.
Participate in training, continuing education, and performance improvement activities to enhance clinical skills and knowledge.
- Experience:
At least 1 year of LPN experience preferred in a healthcare or clinical setting. - Licensure:
Current and unrestricted LPN license in the state or a Compact State License.
- Basic Life Support (BLS) certification required.
- Current CPR certification.
- Skills:
Strong communication, organizational, and interpersonal skills. Ability to work effectively in a collaborative, team-oriented environment. - Physical:
Ability to lift, move, and assist patients as needed.
- Experience in long-term care, rehabilitation, or acute care settings is a plus.
- Knowledge of electronic health record (EHR) systems.
- Bilingual skills may be preferred but not required.
- BLS (Basic Life Support) certification is required.
