As a Hospice Medical Social Worker
, you’ll walk alongside patients and families during one of life’s most vulnerable and sacred chapters, providing emotional support, guidance, advocacy, and compassionate care. This role is more than a job—it’s a calling.

Your Responsibilities
  • Conduct initial and ongoing hospice psychosocial assessments for patients and families.
  • Collaborate with the interdisciplinary hospice team to develop, implement, and update the Plan of Care.
  • Provide skilled social work interventions in accordance with physician orders, professional standards, and hospice regulations.
  • Communicate changes in patient condition, safety concerns, and care needs to leadership and care team members.
  • Support patients and families with counseling, education, and access to community and financial resources.
  • Maintain accurate, timely documentation and ensure compliance with CMS, Medicare, and payer requirements.
  • Implement strategies to promote patient safety and reduce avoidable hospitalizations or ER visits.
  • Uphold Elara Caring’s mission while maintaining strict HIPAA privacy and confidentiality standards.

What You’ll Bring
  • Master’s or Doctoral degree in Social Work from a CSWE-accredited program.
  • Current LMSW or LLMSW/LiMSW, per state licensure requirements.
  • Minimum of 1 year of social work experience in a healthcare setting (hospice experience preferred).
  • Strong assessment, counseling, advocacy, and care coordination skills aligned with professional social work standards.
  • Ability to travel locally (approximately 50% field‑based travel).
  • Valid driver’s license, reliable transportation, and current auto insurance.
