Inter Care Community Health Network in Benton Harbor, MI is seeking a full-time Care Coordinator to join our team. The role focuses on population health management, care navigation, and coordinated services for patients and families.
Starting pay is $19.00 per hour with a comprehensive benefits package including PTO, holidays, tuition reimbursement and health, dental, vision, life, and disability insurance. Normal schedule is Monday–Friday, 8:00 a.m. to 5:00 p.m., with occasional variations.
