Position Summary:

As an Independent Optometrist leasing space inside Sam’s Club, you will take a leading role in patient care with respect to eye and vision care as well as general health and well‑being. You will have autonomy to set your days, hours, schedule, fee schedule and keep 100% of exam/professional fees.

• Establish days of practice
• Establish hours of operation
• Control patient schedule
• Set fee schedule
• Keep 100% of exam/professional fees

• Equipment/instrumentation support
• Administrative and/or Opt‑Tech support services (if state allows)

• Doctor of Optometry (OD) from an accredited optometry school
• Valid state optometry license
• Doctor of Ophthalmology (MD or DO) from an accredited ophthalmology school
• Valid state ophthalmology license
• General Liability Certificate
• Professional Liability Insurance Certificate
• W‑9 form
