- Setting: manufacturing facility in Berea, KY
- Patient ages: 18+ (no dependents, spouses, or children)
- Primary care setting with chronic condition health management
- Acute Care visits
- 10-15 ppd or less
- Schedule:
Tuesdays 8:30-2pm
- EPIC experience
- Active KY license
- 1099 contract
- 100% of malpractice insurance covered
