Anticipated Territory:
Berkeley Area.

Schedule:

Full days every Saturday & Sunday (4 SOCs/day). Full‑time options are available for Saturday & Sunday plus three weekdays.

• Conduct Start of Care (SOC) admissions - OASIS
• Perform follow‑up and evaluation visits as needed within the designated territory
• Make home visits to patients in the Berkeley Area
• Reconcile medication, wound care, treatments, and procedures
• Monitor client conditions and report changes to the clinical team
• Collaborate with physicians and agency staff to deliver optimal care
• Initiate and administer IV medication (antibiotics and diuretics) in accordance with protocols
• Accurately document observations, interventions, and evaluations of patient care management

• Current and unrestricted California RN license
• Current BLS/CPR certification
• Valid California driver’s license
• Experience conducting assessments and Start of Care (SOC) – OASIS required
• High level of clinical knowledge in home health care preferred
• Experience with high‑acuity patients is a plus
• Experience working with an interdisciplinary team highly desired
• Case Management fundamentals highly desired
• Previous experience with IV medication infusions highly preferred
• Proficiency initiating intravenous (IV) therapy is a plus
• Self‑directed, empathetic, critical thinker, excellent interpersonal skills, leadership skills, organized
