We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our Wellness team. The LPN will provide hands‑on resident care, administer medications, maintain accurate documentation, and work closely with the Registered Nurse and care team to support residents’ health, safety, and well‑being.

• Provide resident care in accordance with individualized service plans.
• Assist residents with activities of daily living as needed.
• Administer prescribed medications and accurately document all services provided.
• Monitor residents for changes in health, behavior, or functional status and promptly report concerns to the Registered Nurse.
• Assist with developing and updating resident service plans and care documentation.
• Respond to resident emergencies, provide first aid, and arrange appropriate medical follow-up.
• Communicate with physicians and other healthcare providers as directed.
• Provide residents with compassionate one‑on‑one support and reassurance.
• Help maintain a safe, comfortable, and respectful environment for residents.
• Support the Registered Nurse with staff training, assignments, and other clinical responsibilities as needed.
• Follow all community policies, procedures, and applicable healthcare regulations.
• Promptly report resident incidents and work‑related injuries.

• Current and active Licensed Practical Nurse (LPN) license in the State of New Jersey.
• Successful completion of an approved practical nursing program and the required licensing examination.
• Strong communication, organizational, and documentation skills.
• Compassionate, dependable, and committed to providing quality senior care.
• Prior experience in assisted living, senior living, or long‑term care is preferred.

• 7:00am-3:00pm
• 11:00pm-7:00am
• Weekends

$25 - $30 an hour
