Geriatrische Klinik St. Gallen AG sucht eine/n Sozialarbeiter/in FH zur Mitwirkung im Bereich der Geriatrie. Sie erfassen die soziale Situation der Patientinnen und Patienten beim Spitaleintritt, beraten Angehörige und Patienten zu Wohn- und Lebensfragen nach Austritt und organisieren die Nachsorge. Zudem leiten Sie behördliche Massnahmen ein.
