Position TitlePatient Care Tech – DaVita Skilled Nursing Dialysis (DSD)

Schedule3‑4 days a week, 12+ hour shifts. Full‑time The PCT will be based at the site dialysis den in the Premier Washington – Oak Springs facility and may support additional surrounding sites as needed. Operations run Monday through Saturday with scheduled days changing based on market needs.
OverviewDaVita Kidney Care is establishing a new team—DaVita Skilled Nursing Dialysis Services—to provide quality dialysis care at skilled nursing facilities (SNFs). Approximately 10% of all dialysis patients reside in an SNF and are transported to dialysis three times a week, creating a significant burden on patients and the health‑care system. DSD teammates will enjoy the resources of DaVita while rapidly building a new service line in a start‑up environment.
Candidates should thrive autonomously and collaborate across the Village.
In this role you will be responsible for the complete ongoing care of each patient, ensuring the dialysis treatment stays on track. PCTs work with dialysis RNs to deliver dialysis care.
ComplexityDSD Dialysis care is complex but fulfilling. Meeting and exceeding clinical outcomes is the goal, and multitasking is expected to meet the needs of the service, patients, and SNF partners.
Autonomy and Critical ThinkingProvide comprehensive patient care independently, monitoring patients before, during and after dialysis, recording stats, patient observations and setting up the hemodialysis machine.
Responsibilities- Deliver full dialysis treatment to each patient, coordinating with RNs and other team members.
- Monitor patients and maintain accurate treatment records.
- Ensure patient safety during all dialysis procedures.
- Collaborate with cross‑functional resources across the Village.
- Adjust treatment plans based on patient status and clinical guidelines.
- High school diploma or equivalent.
- Physical and mental ability to work long hours, some shifts up to 12+ hours, on your feet.
- Healthcare experience is a plus; certifications such as BON, Certified Hemodialysis Technician (CHT), Certified Nursing Assistant (CNA), Certified Medical Assistant (CMA), or Phlebotomy are desirable.
- Basic computer skills and proficiency in MS Word and Outlook.
- Training, teaching or healthcare education background.
