We are seeking an experienced Nurse Practitioner specializing in Geriatrics to join our team serving at a Continuing Care Retirement Community (CCRC). This position offers the opportunity to practice in a setting dedicated to the comprehensive care of older adults, working alongside a collaborative interdisciplinary team highly trained in geriatrics and senior care.

In this role, the Nurse Practitioner will:

• Provide ongoing medical management and acute interventions for patients in post-acute care settings.
• Develop, update, and evaluate medical treatment plans, medications, and other necessary care.
• Work as a member of the interdisciplinary team to establish processes that address the medical, psychosocial, and spiritual care needs of each resident.
• Demonstrate advanced clinical knowledge, technical competence, and sound clinical judgment in managing complex geriatric patients.
• Foster trusting relationships with patients, families, and staff within a supportive, team-based environment.
