LPN (Licensed Practical Nurse) - Evening Shift

We are seeking a compassionate and skilled Evening LPN to join our team at Valley Manor Rehabilitation and Healthcare Center in Coopersburg, Pennsylvania. As a valued member of our healthcare team, you will play a critical role in delivering exceptional patient care and supporting our mission to "place our patients on the rapid road to recovery."

• Provide direct patient care, including administering medications, performing vital signs, and monitoring patient conditions
• Develop and implement individualized care plans that address the unique needs and goals of each patient
• Collaborate with the interdisciplinary care team to ensure comprehensive care and coordination of services
• Document patient care and progress in accordance with facility policies and regulations
• Participate in quality improvement initiatives and contribute to the development of best practices
