Occupational Therapist (OTR/L)
Valley Manor Health & Rehabilitation Center is seeking a compassionate, motivated, and licensed Occupational Therapist (OTR/L) to join our rehabilitation team.
For more than 50 years, Valley Manor has served the local community by providing high-quality post-acute care, short-term rehabilitation, skilled nursing, and long-term care services. Our interdisciplinary team works together to help each resident achieve the highest possible level of independence and quality of life.
Responsibilities- Evaluate residents’ physical, cognitive, and functional abilities
- Develop and implement individualized occupational therapy treatment plans
- Provide therapeutic interventions to improve activities of daily living, mobility, coordination, strength, and independence
- Educate residents, families, and caregivers on treatment plans, adaptive techniques, and equipment use
- Monitor resident progress and adjust treatment plans as needed
Complete accurate and timely clinical documentation - Participate in care planning meetings and interdisciplinary team conferences
- Recommend adaptive equipment and environmental modifications
- Collaborate with physicians, nursing staff, therapists, and other members of the care team
- Ensure services are provided in accordance with facility policies and applicable state and federal regulations

- Current and active Occupational Therapist license in the state
- Graduate of an accredited occupational therapy program
- Current NBCOT certification
