Senior Children and Young People's Social Prescriber
Yorkshire Health Partners are looking to appoint an experienced Senior Children and Young Persons Social Prescriber to support the delivery and development of our high-quality Children and Young Persons Social Prescribing Service.
On a day-to-day basis you will empower young people between the ages of 11-18 to take control of their health and wellbeing through your skills in social prescribing - giving time and a safe space for them to have a focus on what matters to me. This holistic approach connects people to community groups and statutory services that provide emotional and practical support for patients their families and carers to take control of their health and wellbeing - live independently - improve their health outcomes and maintain a healthy lifestyle.
Mainduties of the job
As well as empowering young people and managing a complex caseload, the Senior Children and Young Persons Social Prescriber will also be responsible for the line management of a team of Young Peoples Social Prescribing Link Workers, provide day to day support and contribute to training and mentoring, maintain accurate records and contribute to monitoring and reporting outcomes, and support quality improvement.
You will take referrals from the Primary Care Network and a wide range of agencies to support the health and wellbeing of patients. You will work closely with GP practices across the PCN, working alongside young people, supporting them to tackle loneliness, isolation, anxiety and depression by connecting them into a broad range of services and interventions to improve their health and well-being.
The work will be led by the wants and needs of the young people themselves and may need some flexibility in working hours to fit availability of the young person.

Job responsibilitiesKey Areasof Responsibility
Toprovide line management for a team of Young Peoples Social Prescribing Link Workers for the primary care network.
Tosupport the Locality Service Manager in the delivery of the Young Peoples Social Prescribing Service.
Toact as a point of contact for young people accessing the service.
Operational Management
Accept referrals and assess how far a patients health and wellbeing needs can be met by service sand other opportunities in the community.
Manage and prioritise your caseload reflecting your patients needs.
Together produce a simple personalised care and support plan to address their health and wellbeing needs.
To ensure young people are made aware of key processes e.g. complaints - information sharing - getting involved and to make sure that they are asked about how they would prefer information.
Following a patients journey evaluate how far the actions in the care and support plan are meeting their health and wellbeing needs.
Where required refer patients back to other health professionals within the PCN.
Financial Management and Planning
Ensure data is captured in the clinical systems and SNOMED codes are utilised.
Gather data to build an evidence base about the benefits of social prescribing.
Building capacity through effective leadership
Undertake performance management discussions - coaching and appraisals for the individuals that makeup the link worker team.
Support the VCSE organisations and community groups to become more sustainable - sharing intelligence regarding gaps or issues identified in local provision.
Ensure all VCSE organisations have appropriate safeguarding procedures in place and are genuine opportunities for patients to develop a sense of belonging - build knowledge - skills and confidence.
Sharing the knowledge and promoting the benefits with colleagues in the PCN on other services available in the local community and how these can be accessed.
