Social Care Coordinator x 3

Fixed Term – 12 months | Full Time

We are excited to offer a opportunity for three Social Care Coordinators
to join our Adult Social Care team 2 x Bridlington and 1 x Haltemprice within the Community well-being teams

About the Role

An exciting opportunity has arisen to appoint 3 care coordinators: 2 posts to join Bridlington and 1 post to join Haltemprice Adult Community well-being team, working on a blended approach which includes working from the office and home.

The role of a care coordinator will be to work with people over the age of 18, their families and carer’s with a key focus on supporting people to remain independent within their community settings.

This will include working with people and carers through the assessment process to identify, build and maintain family and social networks, to support and enable people and carers to actively stay well and independent, in line with strength based approaches a key in promotion of independence, wellbeing and choice.

About You

We are seeking a suitably qualified, enthusiastic and resilient individual who can:

  • Work confidently with colleagues at all levels and across multiple organisations.
  • Communicate clearly, professionally and with compassion.
  • Think on your feet, respond to changing situations and manage competing pressures.
  • Demonstrate an understanding of key legislation including the Care Act 2014
    , Mental Capacity Act 2005
    , Human Rights Act
    , and safeguarding procedures.
  • Work independently, manage time effectively and maintain high-quality written and IT standards.
  • Build positive relationships that strengthen partnership working and improve outcomes for adults.

Access to transport and a full, clean driving licence is essential due to travel between locations.
