Responsibilities

Assess and Adapt: Each visit starts with evaluating the patient’s current condition, medical needs, and overall well‑being.

Create and Modify Care Plans: You’re not just following orders—you’re actively shaping your patients’ treatment plans based on what they need, adjusting care as conditions change.

Teach and Support: Whether it’s a psychiatric patient needing medication guidance or a post‑op patient learning about wound care, you play a vital role in helping them stay independent at home.

Communicate and Coordinate: If something changes, you keep providers, families, and care teams informed, ensuring patients get the right care at the right time.

Document and Reflect: At the end of your day, you submit your notes from home, ensuring every visit is recorded accurately, not just for compliance but for the next nurse who continues their care.

Qualifications
  • Active Registered Nurse (RN) license in Massachusetts.
  • One year of nursing experience (home health or psych preferred).
  • A drive to provide compassionate, patient‑centered care.
  • Reliable transportation for patient visits.
