Qualified Social Worker - Adult Triage

This role is for a Social worker to work at the front door of adult social care in an integrated health and social care team. Assessments, Mental Capacity assessments, Duty rota worker, telephone triage, Safeguarding concerns and enquiries.

Fast and dynamic team of Social Workers, Social Care Assistants, Occupational Therapists, Nurse and Physio.

Requirement to be able to act fast in a crisis, complete dynamic risk assessments, have excellent interpersonal skills and be a team player.
