London Borough of Bexley is seeking a Social Worker for the Leaving Care Service to support 16 and 17-year-olds as they transition to independence. You will conduct assessments, develop care plans, and advocate for young people’s rights within a robust, person-centred pathway framework.
You will work in a case-holding role, coordinating multi‑agency support and ensuring statutory duties are met. The role requires DBS clearance and office presence at least three days a week.
