Community Care Social Worker/Case Manager (Full Time)

• Monday through Friday – remote option (must live in Yellowstone or surrounding counties)
• No call or weekend shifts

Works in a multi‑disciplinary approach to plan, coordinate, monitor, and supervise the provision of services to consumers enrolled in the Home and Community Based Services Program.

• Plan, coordinate, and monitor services for Home and Community Based Services Program participants.
• Handle pressure, meet deadlines, maintain accuracy, and adapt to changing situations.
• Interact with a variety of people, manage stressful situations, and resolve conflicts.
• Perform assigned job duties, adhere to licensing board regulations, and follow Benefis policies and procedures.

• Bachelor’s degree in social work, psychology, sociology, or a related field.
• Three (3) years of social work experience in a health care setting (preferred).
• Knowledge of case management methods, diagnostic and crisis intervention skills.
• Knowledge of issues and needs of long‑term care consumers.
• Experience managing a budget.
• Understanding of human behavior, disabilities, and the aging process.
