American Traveler is hiring an experienced RN Case Manager for a hospital care management role requiring 3+ years of acute care nursing experience and a BSN.
Details- Hospital Care Management unit within an acute care hospital setting
- Day shift, 8-hour shifts, 5 days per week, 40 hours total
- Coordinates care and services for patients meeting Care Management criteria
- Uses Epic for charting
- Active RN license (Montana or Compact)
- BSN required
- BLS certification required (AHA, not expiring prior to start)
- Minimum 3 years of RN experience in an acute care setting required
- Hospital care management experience required
- 1 year of experience in case management specialty required
- Knowledge of medical terminology required
- Proficiency with desktop software including PowerPoint and Excel required
- 5 years of nursing experience preferred
- ACM or CCM certification preferred but not required
- Coordinates patient care and discharge planning based on comprehensive assessment of medical, functional, and social needs
- Collaborates with physicians and multidisciplinary care team to develop and execute individualized care plans
- Identifies strategies to reduce length of stay and improve appropriate utilization of services
- Monitors outcomes through variance tracking and data collection
- Educates physicians and ancillary staff on case management and discharge planning processes
- Advocates for patient rights and participates in rounds, case conferences, and multidisciplinary groups
- Returning staff must be separated from any Intermountain facility for at least one year before eligible for a travel assignment
