10,000 sign-on bonus with 30-month commitment.

Days ), 3x12, 1 on-call day shift per 6-week rotation.

The Rehab LVN/LPN is responsible for providing direct patient care through assessments, nursing interventions, documentation, and interdisciplinary coordination, ensuring the highest standards of safety and quality.

• Provide direct patient care, including medication administration, treatments, and monitoring of patient conditions.
• Assist patients with activities of daily living (ADLs) and support mobility and rehabilitation goals.
• Observe, document, and report changes in patient status to the interdisciplinary care team.
• Collaborate with physicians, nurses, therapists, and case managers to coordinate patient care.
• Educate patients and families on treatment plans, medications, safety, and discharge instructions.
• Maintain accurate documentation and promote a safe, therapeutic environment.

• Applicants should have a current Montana LPN/LVN license or Compact LPN/LVN License with Montana endorsement, and Graduation from an accredited licensed practical/vocational nurse program.
• Certifications:
  
  CPR/BCLS required; IV Certified at time of hire or obtained within 6 months of hire required; ACLS preferred
• Experience:
  
  Minimum 6 months Medical/Surgical experience in an acute care setting and 6 months in a Rehabilitation setting preferred.
